The American Academy of Facial Esthetics (AAFE) Training Center Cleveland is looking for good candidates to be model patients for our Botox®, Dermal Filler and PDO threadlift continuing education training.
As a model patient in a training course, you receive the same high-quality Botox® and Filler treatments as in a professional medical spa, but at 50% – 70% off retail.
Services are performed by trained and supervised licensed healthcare professionals who are either physicians, dentists, nurse practitioners, nurses, and physician assistants. This treatment may take more time as this is a learning process which is important for treatment success.
Model patient requirements:
- Be between the ages of 21 to 50 years of age
- Good overall health
- Comfortable with receiving injections
- Allow photos and videos to be used for social media, marketing, and educational purposes - treatment will be taped
- Follow up appointment required so your results can be measured

If you are interested in being accepted as a model patient, please complete the form below, and submit two close up photos of yourself, one with your face at rest and one of you smiling. These close up photos should show from the top of your head to the bottom of your chin. You will be contacted by a member of our team to let you know if you are a good candidate for the AAFE training center.
PLEASE NOTE: We require a 48 hour cancellation notice and charge $50 for no-shows and late cancellations. Appointments are considered no-shows 15 minutes past their scheduled appointment time. We do not take a $25 booking fee, but we do require a valid credit card to book an appointment.
